We're offering a deserving veteran the chance to receive a brand-new roof, completely free of charge, in the Kokomo, Westfield or Frankfort, IN area. If you or a veteran you know is dealing with an old, worn-out roof and financial challenges, we invite you to nominate them today.

Please complete all sections to the best of your ability. Incomplete applications may delay processing.

• Copy of DD Form 214 (Verification of Military Service)
• Proof of Ownership (if applicable)
• Proof of Income (if applicable)
• Photos of roof damage (optional, but highly encouraged)

Participation is contingent upon proof of veteran status, which must be provided upon request.
